NNDC Clinical Care Registry (CCR) Data Set

NNDC established the Clinical Care Registry (CCR) in 2010 and collected data through the end of 2014.  The CCR was designed to utilize the NNDC’s Screening and Monitoring Package to implement a long-term, prospective, observational, multi-center patient registry.

The CCR collected data in research studies and routine clinical care from adult, literate, English-speaking patients with depression, bipolar, anxiety disorders, PTSD or related conditions, across the participating NNDC Centers of Excellence.

The Screening and Monitoring Package includes two categories of data:
  1. Baseline Assessments: completed at the initial evaluation of each patient participating in the program. Includes enrollment, demographic, and clinical assessment of the patient’s condition and treatment history plus patient-rated assessments.
  2. Continuing Assessments: completed at follow-up visits for each participating patient. Includes the patient-rated assessments for longitudinal monitoring.

The Screening and Monitoring Package consists of the following patient-rated assessments:
  1. PHQ-9: Patient Health Questionnaire - A 9-item depression symptom severity scale.
  2. GAD-7: Generalized Anxiety Disorder Assessment - A 7-item scale assessing the degree of anxiety experienced by the patient.
  3. ASRM: Altman Self-Rating Mania Scale - A 5-item self-rating scale to assess the severity of manic symptoms in the past week.
  4. C-SSRS: Columbia Suicide Severity Rating Scale – Patient-Rated - A scale designed to assess the presence of suicidal thoughts and self-harming behaviors

The NNDC CCR Data Set includes data on 1,500+ patients presenting with a depressive episode from 18 participating NNDC Centers of Excellence.
